YOUR SIGNATURE AND NAME MUST BE THE SAME AS ON YOUR EXPIRED PASSPORT.

PLEASE NOTE:

A barcode MUST appear on the top left portion of your application for it to be valid.

B. Current passport must be submitted (It will be returned along with the second passport.)

C. Two passport-size photographs (2x2) - Pictures available at Walgreens, CVS, etc.

D. Proof of departure. One of these:

Copy of airline tickets
Copy of itinerary or reservation
Letter on Company's Letterhead (job or social institution)

E. Letter of authorization (Click Here)

This is a basic release form that allows Passport Nation Inc. to hand carry your documents to the passport agency.

F. Government Fees:

U.S. Passport Process

3–5 business day Service $800

Add $50.00 shipping fee (FedEx/UPS) if required.

STEP 3. Ship your documents along with the Passport Nation order form and fees.

You will place the signed application (DS-82), your expired passport, your 2 passport photos, the proof of departure, the letter of authorization, the government fees of $190.00, the Passport Nation Order Form, and Passport Nation fees (if paying by money order) inside the shipping envelope (FedEx, UPS, or DHL.)

For credit card payment, please complete the credit card information on the order form. We accept Amex, Visa, Mastercard, and Money Orders.
