Comprehensive Passport Renewal Services

STEP 1. Gather the following requirements:

A. Application Form DS-82 (Click Here)

Application Form - Must be completed online and printed here. Please make sure that the name on the application matches the name on the most recent passport.

A barcode MUST appear on the top left portion of your application for it to be valid.

B. Most recent US passport book (If a passport card has been issued as well, you will need to submit it).

Must have been valid for a total of 10 years and cannot be expired for more than 5 years.

C. One passport-size photograph (2x2) - Pictures available at Walgreens, CVS, etc. Must be against a white background without glasses.

D. Proof of departure One of these (Only required if the passport is needed in less than 6 business days):

Copy of airline tickets
Copy of itinerary or reservation
Letter on Company's Letterhead (job or social institution)

E. Letter of authorization (Click Here)

This is a basic release form that allows Passport Nation Inc. to hand-carry your documents to the passport agency. Please list the applicant's name in the order requested, your phone number, put the current date (the date must match the same date as on the signed DS-82 application), and the applicant's signature at the bottom. Please fill it out completely by hand in black ink.

F. Government Fees:

Personal check with name and address preprinted or money order made out to the US Department of State for $190.00 (or $220.00 if you would like to add the passport card).

(TEMPORARY CHECKS ARE NOT ACCEPTED.)

STEP 2. Complete an Order form
